Financial Agreement
Our policy requires payment in full for all services rendered at the time of service. If we are filing your dental insurance, you will be expected to pay the estimated patient cost of what insurance will not cover according to your policy.
Insurance Disclaimer
Any insurance estimates provided by our office are estimates only and are not a guarantee of payment. Insurance companies determine benefit eligibility and payment at the time the claim is processed, not at the time of treatment.
Dental insurance is a contract between the patient and the insurance carrier. Patients are fully responsible for understanding their benefits and for payment of all charges not covered by insurance.
If a balance is due after all insurance payments are processed, the balance must be paid within 90 days. If an amount is not paid within 90 days of the date of service and no financial arrangements have been made, your account may be turned over to collections as well as finance/billing charges will occur. You will be responsible for legal fees, collection agency fees, interest charges, and any other expenses incurred in collecting your amount owed on the account.
*Our cancellation policy requires a full 24-hour notice for all cancelled appointments. After the second missed appointment where no notice is given, a $50 fee will be charged.
